PERISPHERA® A – Anal probe for incontinence rehabilitation
The probe is intended for the treatment of male patients with post-prostatectomy urinary incontinence or patients with defecatory disorders. The depth of insertion is adjustable, from 40 to 55 mm, according to the anatomical structure of the patient, by means of the safety hilt with 4 positions which also determines the correct orientation of the probe. The 4 electrodes (2 front and 2 rear) can be connected to separate output channels of the electrostimulation unit or to a single channel (through special “Y”-shaped optional wire adaptors).

Characterised by two spherical electrodes, it is intended for global, undifferentiated and simultaneous stimulation of the puborectalis and both branches of pubococcygeal muscles, regardless of the orientation of insertion into the vagina.

With two egg-shaped electrodes and predominant insulation on the top, it is intended for the global stimulation or the EMG biofeedback of the pelvic floor excluding the upper wall adjacent to the bladder. In the treatment of patients with asymmetric perineal muscles following episiotomy or lacerations the PERISPHERA®-E probe allows selective treatments. By rotating the probe 90 degrees to the left or to the right, you can apply a one-sided and independent treatment to each of the two branches of the pubococcygeal.

The probe is characterised by a double circuit formed by two large flat electrodes (lower side) for the global stimulation of the pelvic floor and two “saddle-shaped” side electrodes for the undifferentiated stimulation of both the right and the left branches of the pubococcygeal muscle. For optimum use it is advisable to utilise both circuits, however if the operator is accustomed using probes only equipped with side electrodes.

Indicated for the treatment of patients with limited vaginal access or suffering from vaginismus. A diameter of barely 11 mm and the particular arrangement of the side electrodes allow an easy stimulation of the perivaginal muscles. The safety hilt for adjusting the depth of insertion and determining the orientation can be removed.

The therapeutic unit evoStim® E is an electromedical equipment for perineal re-education by electrostimulation and EMG biofeedback. It allows passive perineal treatments (with 1 electrostimulation channel), active perineal recovery (with 1 biofeedback channel) and perineal muscular re- learning (by EMG-triggered electrostimulation).
It is intended for both professional use and personal use by the patient.
Perineal electrostimulation
(programmes URGE-STRESS-MIX-PAIN)
In personal use, the rapid selection of a perineal electrostimulation program, among the 4 available, is intuitive for the patient and, when the unit is switched-ON, the last program previously used is loaded.
The pulse generator can deliver symmetrical biphasic pulses (for ring electrode probes) or alternating biphasic pulses (for side-electrode probes).
EMG Biofeedback
Biofeedback is of EMG type, and can be applied either on perineal muscles (with vaginal / anal probe) or on external muscles (with transcutaneous self-adhering electrodes). Since the acquired EMG signal is in the order of a few μV, EMG biofeedback also requires the use of a transcutaneous reference electrode, essential for the suppression of electrical interference.
Visual feedback on the display consists in switching-on a number of concentric circles proportional to the pressure exerted on the balloon of the probe by perineal muscles, during a voluntary contraction. With the range self- calibration, administering biofeedback programs is quite easy with the evoStim® E unit.
Evoked stimulation (programme EVO)
Evoked stimulation, also called muscular cognitive re- learning, Evoked stimulation exploits both, the active re-education (biofeedback) and passive re-education (electrostimulation). The EVO program provides action/rest cycles of electrical stimulation, but any cycle must be triggered by the voluntary contraction of the patient when a certain threshold is attained. The acquired EMG signal is measured in μV and it is directly proportional to the contraction of perineal muscles, which may be measured in the vagina. The pre-defined trigger threshold is 8 μV and may be modified by the user. The threshold also changes automatically, increasing every 3 valid voluntary contractions. The last reached threshold value is automatically stored at the end of the session and will be the start threshold of the next session. Patient’s ability, his/her willing and perseverance to reach ever greater thresholds makes this program involving and effective for functional recovery of the pelvic floor.

Description
evoStim® UG is a dual channel electrical stimulator specifically designed for perineal stimulation with vaginal or anal probes. It is ease to use without sacrificing the flexibility and performance. The touch-screen and the unique rotating knob offer a friendly interface either for professional or home use. The intensity of stimulation and the waveform are adjustable independently on the two channels, offering the possibility of using probes with 4 electrodes of different shapes and positions.
The ergonomics of evoStim® UG unit is based on the rotation of the upper knob (to adjust the stimulation intensity or the value of the various parameters) and the push of the button, integrated in the same knob, to turn the unit on / off or pause / resume it. Changing the stimulation intensity or any other parameter must first be enabled by tapping the appropriate area of the touch screen.
Intended use
The evoStim® UG unit can be directly used by the patient, but it is also suitable for professional use by health professionals dealing with physiotherapy and rehabilitation. Perineal electrical stimulation (PES) is commonly used to treat urinary incontinence in its various forms: urge, stress and mixed.
50 programmes for perineal electrostimulation
The 5 virtual buttons at the bottom of the screen allow to quickly select one of the 5 therapeutic aims for the treatment of incontinence (URGE, STRESS and MIX) or for the treatment of pelvic pain (PAIN) or for the treatment of vaginism (RELX).
Each one of the 5 quick-selection virtual buttons is associated with a “standard” program, which is supported by the automatic parameter setting function (ACTION / REST) if the stimulation frequency is changed.
Therefore, since the last program used is re-loaded when the device is switched-on again, the simplified direct use for the patient only requires:
switch-ON the unit,
select one of the 5 virtual buttons (for the first session only),
adjust the stimulation level for one or both channels.
Instead, the professional can link a different program (each of 10 for each button) to each of the 5 virtual buttons and all the parameters can be permanently changed. At any time, however, you can reset to the factory settings a single program, a program group, or all programs.

Since the electromyographic detection requires a reference electrode, it is necessary to apply a trans-cutaneous adhesive electrode on the outer surface of the skin, preferably at a bony prominence (eg: iliac crest). The same probe can be used for pressure biofeedback (also simultaneously with EMG) by detecting, through the balloon, the global pressure of the pelvic floor, which is directly proportional to the force produced by the perineal muscles. In some cases, the use of the manometric biofeedback by the VAG-2STFW probe is easier than with the EMG biofeedback, because it is not affected by hormonal factors of the vaginal mucosa. Furthermore, manometric biofeedback does not implies any external reference electrode.The balloon is made of medical silicone. For the pressure biofeedback, the probe must be used with manometric biofeedback equipment, using a standard Luer-lock pneumatic connection (ISO 594). The probe does not require to be pre-inflated; only precaution, connect the probe to the biofeedback unit, before insert it into vagina.
Patient position for use
It is suitable to treat a patient in laying position, the particular construction makes it easy to introduce and comfortable to be retained by the patient

The two asymmetrical circular electrodes, the upper one (or distal), which is the wider, and the lower one (or proximal), which is smaller, spaced by 13 mm each other, imply the undifferentiated stimulation of all perineal muscles. The electrodes profile is interrupted at the top, for the purpose of avoiding the stimulation at the upper side of the probe, bordering the bladder.

The two large surface, circular electrodes, distal and proximal, integrated into the probe stem, cause undifferentiated stimulation of the pubo- rectal muscle and the anal sphincter.
A large plastic ring, at the external end of the probe, divides the region in contact with the patient from the region aimed to electrical connections. This is obtained by a couple of extra-flexible wires, terminating with 2 mm female connectors.

Due to the position and the size of electrodes, the probe ANA-2ST is not suitable for EMG biofeedback.

Intended use
TOTEM-UG is a patient’s wearable professional unit for the re-education of incontinence by electrostimulation and / or perineal biofeedback, through wireless connection (wireless) with a personal computer. Therapeutic goals are the attenuation or remission of incontinence, exploiting the effectiveness of perineal stimulation and the great contribution of biofeedback techniques to the
functional re-learning. The TOTEM-UG unit is applied to the waist of the patient by means of the belt enclosed in the package.
The use of TOTEM-UG is intended for healthcare professionals trained for perineal incontinence rehabilitation (gynaecologists, urologists, physiatrists, physiotherapists, obstetricians).
The special probe TOTEM Combicon
It allows to:
1 – perform the asymmetric electrostimulation of the two branches of the pubococcygeus muscle with independent parameters and intensities and, simultaneously, acquire a pressure value proportional to the global contraction of perineal muscles.
2 – Simultaneously acquire the EMG signals produced during the voluntary contraction of the two branches of the pubococcygeus muscle and, simultaneously, acquire a pressure value proportional to the global contraction of perineal muscles.
Electrical stimulation and EMG Biofeedback
Electrical stimulation is performed by symmetrical biphasic pulse generators (or alternating biphasic pulses, in case of probes with lateral electrodes), with controlled energy.
In addition to the probes included in the package or supplied as optional accessories, the unit may be used with any type of perineal probe with at least two electrodes and 2mm female connectors.
Pressure Biofeedback
In addition to the EMG biofeedback, the manometric intra- cavity biofeedback can be performed, with a probe VAG-2STFW or any type of balloon probe equipped with Luer-lock female pneumatic connection. With the double biofeedback channel, it is possible to acquire and show on a monitor an intra-cavity pressure signal (vaginal probe) and, simultaneously, a surface EMG signal with transcutaneous electrodes applied over abdominal muscles. This procedure is suitable for the treatment of perineal dysfunctions linked to the so called “inversion of command”.
